Each day, all athletes will be required to use their phone to complete the CCISD COVID-19 SELF-HEALTH ASSESSMENT. If they don't have a phone, then they will need to complete the form with their assigned coach. The Athletic Trainers will participate in this process. All athletes are required to bring their own water. Each athlete is encouraged to bring a jug of water. A gallon jug is suggessted. The jug will need to be labeled with their first and last name.

All camp participants must have a physical on file with our Athletic Trainers. Last school year's physical is sufficient for Summer SAC Camp. In and for the consideration of my son’s/daughter’s participation in the Clear Creek Summer SAC Camp, I hereby agree and promise I will not hold the Clear Creek Camp nor its employees responsible for any loss, damages, or personal injuries he/she may receive as a result of participation. Clear Creek Independent School District does not carry insurance for summer fitness/recreation programs. Parents will be responsible for any medical expenses incurred. Completion of camp registration means that this medical release has been read and agreed upon with the guardian signature on the registration form.
